The review was mostly favorable, and, as I consider applying for other jobs, I'm wondering if it's considered appropriate to include this written evaluation with other application materials I may send out. Is it even appropriate to include excerpts from the review in a cover letter?

An honest, detailed and generally favorable evaluation can be a great tool in landing a new job. It is entirely appropriate to show that to a prospective employer.
The question is timing.
You seem appropriately reluctant to send it out with your application. It is better saved for the reference-checking stage and can be especially helpful if you don't want people calling your present employer.
It is fine to quote from your evaluation in an application letter, but I would limit it to one or two carefully chosen phrases.
